How to Apply for OFW Dependent Scholarship Program?
To apply for the OFW Dependent Scholarship Program, aspiring candidates must meet the following Documentary Requirements:
- Scholarship Application Form – OWWA RWO
- OFW Membership Record – OWWA RWO
- 2 copies of ID Pictures with Name tag (Government Issued or Company Issued ID Applicant)
- At least 1 Valid ID of the OFW member
- Authorized Representative Government-issued or a Company ID
- Proof of Relationship of the OFW Member which can be:
- Birth Certificate from (PSA) – Philippine Statistics Authority or (LCR) – Local Civil Register
- If the Dependent is Sibling – OFW Birth Certificate
- OFW Cenomar if single – Dependent’s Birth Certificate
- Form 137 Certified true copy of Form 138
- TOR – Transcript of Records indicating 80% GWA Without any failing Grades
- Certificate of Good Moral
- Medical Certificate:
- Physically fit to attend School if the Student has a Medical Pre Condition’s
Candidates are required to upload initial requirements with a file size limit of 3 to 4 MB:
- Proof of relationship (birth certificate if the applicant is a child of OFW, birth certificate, and CENOMAR of OFW if the applicant is a sibling)
